Form 1042-S

An international student or employee will receive a 2025 Form 1042-S if:

1. You submitted Form 8233 or Form W-9 for 2025 to apply for exemption from income tax withholding on compensation in accordance with a tax treaty between the U.S. and your country of tax residence
2. You have received taxable scholarships/grants/awards.

You will need Form W-2 and/or Form 1042-S in order to prepare your U.S. tax returns for 2025.

Note: If you opted to receive an electronic copy of Form 1042-S prior to the 2025 Form 1042-S mailing date, you will receive an email once this form is uploaded to your FNIS account and you will not receive a physical copy in the mail.

Duplicate 1042-S

To request a duplicate 1042-S:

1. Fill out the Duplicate 1042-S Request Form
2. Email the completed form to fntax@nd.edu
