Form W-2
All 2011 W-2 forms were mailed on January 25, 2012.
Form W-2s were mailed to your home address on record as of January 13, 2012. If you need to update your address:
- Faculty/Staff: Obtain a form under category Information Changes from Human Resources at http://hr.nd.edu/forms.
- Students: Obtain a form from the Registrar at http://registrar.nd.edu/addresschanges.shtml
Please note that all address change requests must be in writing. To verify your address is accurate, go to the My Resources Tab on insideND. If you changed your address after January 13, 2012, your 2011 Form W-2 may have been returned to Payroll Services as undeliverable.
Should you need an additional copy of your 2011 Form W-2, see notice below about on-line W-2s available to current employees via insideND, or you may request a duplicate in writing via the completed form below:

The University continues to mail paper W-2 forms to all employees. Shortly after W-2’s are mailed, an online version will also be available. This may be helpful to those employees who lose their original W2. Simply go to the My Resources tab, Employee Personal Information channel and click Tax Information then click on the W-2 link to view your form. Use the print button on your browser to print your W-2 in a format that can be used for filing paper versions of federal and state tax returns.
In order to access your W-2 from an off campus location, you will first need to establish a VPN (virtual private network) connection. Detailed instructions for connecting to the VPN are available at secure.nd.edu/stay-secure/vpn.shtml. If you need assistance connecting the inside ND or the VPN, please contact the OIT Help Desk at 631-8111 or oithelp@nd.edu.

Form 1042-S
The 1042-S forms were mailed on Monday, February 13, 2012.
You will receive a 2011 Form 1042-S ONLY if you submitted Form 8233 or Form W-9 for 2011 to apply for exemption from withholding in accordance with a tax treaty between the U.S. and your country of tax residence. You will need Form W-2 and/or Form 1042-S, if applicable, in order to prepare your U.S. tax return for 2011.
New Payment System Rules for Electronic Payments
The US Treasury has changed payment system rules for electronic payments. If you receive a payment at a US bank and forward the entire amount to a bank in a foreign country, you must advise Payment Services at 574-631-7575. Failure to do so may result in delay or rejection by the payment system.

- Timecards for hourly paid employees due by each Monday at 9:00AM. Non-Exempt Timecard guidelines and procedures click here.
- Report of Absence/Overtime for non-exempt salaried employees due ten (10) days before each pay date. Note: This does not apply to hourly employees.
- W-4s claiming "exempt" status expire February 15th each year. New exempt W-4s are due by February 1st in order to be processed by the Friday 15th Payroll expiration date.
